Start here: the number that sets your budget is not the number in your waste register
A hospital that records a given tonnage of waste per day does not need an incinerator sized for that tonnage. Most of what a hospital, a medical or nursing college, a diagnostic laboratory or a common biomedical waste treatment facility handles in a day is not incinerable at all, and a substantial part of what is correctly classified as biomedical waste is directed by the Bio-Medical Waste Management Rules to treatment routes other than incineration.
I have been quoting incinerators out of our own works since the early nineties, and the single most common defect I see in a biomedical waste tender is a capacity figure derived from total waste generated. It produces a machine several times larger than the site can ever feed. Oversizing costs money twice: once at purchase, and then every day afterwards in fuel burned to bring an over-large chamber up to working condition for a charge that half fills it. A unit that is run far below its intended duty is also harder to operate consistently, which is exactly the wrong outcome on a stream that an inspector will ask about.
So the honest order of work is: segregate, weigh, identify the incinerable fraction, confirm the requirements that apply to your site with your State Pollution Control Board, and only then write a specification. Segregation is the decision that governs everything downstream
Biomedical waste in India is governed by the Bio-Medical Waste Management Rules. Those Rules define the categories of waste, the containers and colour coding used for each, the storage and transport conditions, the treatment route assigned to each category, and the records the occupier is required to maintain. The Central Pollution Control Board issues guidelines that sit alongside them, and your State Pollution Control Board is the authority that applies them to your site.
I am deliberately not reproducing the category table, the colour codes or the treatment parameters on this page. Those provisions are amended, and a buyer who plans a purchase around a version copied from a supplier website is planning around something that may no longer be current. Work from the text of the Rules as they stand today and from what your State Pollution Control Board tells you applies to your institution and your waste stream. That is not a disclaimer, it is the only workable method.
What matters commercially is the consequence: the incinerator you buy treats one fraction of the waste your institution produces. Improve segregation at the ward, laboratory or theatre level and the incinerable fraction goes down, the fuel bill goes down, and the size of machine you needed in the first place goes down. Let segregation slip and you push general waste and recyclables into a stream that costs the most to treat. How to arrive at a defensible capacity
Do a weighing exercise. Segregate to the categories the Rules define, weigh each stream separately over a period long enough to cover your real variation, and record it. A teaching hospital has an academic-year pattern. A diagnostic laboratory has a weekly one. A common facility has to model the collection round it actually serves, not the one in its licence application.

Write the margin and the buffer assumption into the tender in words, not just in the capacity figure, so that every bidder is sizing against the same statement. What to specify, and what each element is actually for
Specify function and construction. Do not specify performance numbers you have taken from a supplier catalogue, because the numbers that carry legal weight come from the Rules and from your consenting authority, and they are the ones a bidder must be told to meet.

Two points I would underline. First, instrumentation and records are not an accessory. When an inspection happens, the record is the evidence, and a plant with no usable recording leaves the operator arguing from memory. Second, air pollution control equipment is where quotations diverge most, so if your tender does not state the required configuration precisely as your authority has given it to you, you will receive quotations that are not comparable and you will end up comparing prices for different machines.
The approvals sequence
Installation and operation of an incinerator is a regulated activity. Consent to establish and consent to operate are issued by the State Pollution Control Board. The order below is deliberately given without durations, because timelines vary by state and by application, and a supplier who quotes you a timeline for someone else’s regulator is guessing.

State this plainly to anyone quoting you: a manufacturer supplies equipment built to the parameters you specify. A manufacturer cannot obtain a consent on your behalf and cannot certify that a machine is compliant in the abstract, because compliance is a property of an installed plant at a site, operated in a particular way, judged by the authority for that state. Any supplier who tells you their unit is pre-approved is telling you something that does not exist. Own installation or a common facility
Where a common biomedical waste treatment facility serves your area, using it is a legitimate answer and sometimes the better one. Treat this as a real decision.

If a common facility is available and your volume is modest, buying an incinerator to gain independence usually does not pay for itself once fuel, manpower, spares and the renewal burden are counted honestly. Documentation for institutional procurement
Government hospitals, medical colleges and autonomous institutions will not accept a quotation on its own. Assemble the pack before the bid closes, not after.
